Removing Bichat Lumps
Removal of Bichat lumps, also known as “cheek reduction surgery,” aims to remove the buccal fat layer from the cheeks. This procedure decreases the thickness of the cheeks and accentuates the contours of the mid-face.
Essence of the Operation
This operation can be combined with other plastic surgeries, such as facelifts, lip augmentation, or rhinoplasty. An incision is made inside the cheeks to remove the fat. Ideal candidates are individuals with disproportionate or excessively plump cheeks. This procedure is not suitable for people with thin and narrow faces or the elderly, as it may lead to an older appearance. It is also not beneficial for those with progressive hemifacial atrophy (Parry-Romberg syndrome). Preparation may include a detailed medical history, medication history, and physical examination.
Duration
The procedure lasts about 30 minutes to one hour.

Local anesthesia with mild sedation is typically used.

Possible Side Effects
After the procedure, patients may experience:

Possible Risks
Potential risks include:
- Excessive bleeding
- Nerve damage leading to paralysis or numbness
- Damage to the parotid duct
- Hematoma (accumulation of blood under the tissues)
- Excessive or insufficient correction
- Asymmetry
- Infection
- Bleeding
- Prolonged edema
- Seroma (accumulation of fluid)
- Need for a repeat operation
Rehabilitation Period
After surgery, it is important to avoid pressure on the surgical area. Patients should stick to a liquid diet for several days, gradually transitioning to a soft diet before returning to a regular diet. A special mouthwash will be provided, which should be used for at least 2-3 weeks to prevent infection. Swelling may decrease within 3-5 days, though daily activities such as eating, drinking, and talking may be slightly limited. Patients can usually return to work within 5-7 days or once the swelling has completely subsided. Intensive activities, such as contact sports, should be resumed 2-3 weeks post-surgery. Full recovery may take at least 3 weeks, during which patients are advised to abstain from smoking.
Results
Final results can be seen in a few months. The outcome is usually stable, but with age, some facial imbalances may occur, affecting the result.
